Refunds Process

Here is an explanation of our refund processes for different senarios.

FOR FULL REFUND OF ALL TREATMENTS AND CAP WAS SHIPPED

A refund can be processed following completion of page 2 of the enrollment form by the treating physician or nurse, which needs to be faxed to the Paxman Hub. Since the Cap has already been shipped, once the Cap is returned, a refund will be processed with a $220 restocking fee applied.

FOR FULL REFUND OF ALL TREATMENTS AND CAP WAS NOT SHIPPED

A refund can be processed following completion of page 2 of the enrollment form by the treating physician or nurse, which needs to be faxed to the Paxman Hub. Since the order has already been sent to the pharmacy, a refund will be processed with a $25 restocking fee applied.

FOR UNUSED TREATMENTS ONLY

The cap is non-refundable.  Any unused treatment cooling credits are refundable following completion of page 2 of the enrollment form by the treating physician or nurse, which should be faxed to the Hub.

Refund Policy: Foundation Patients

The cap is only refundable if tamper-seal box remains unbroken.  Any unused treatment is refundable following completion of page 2 of the enrollment form by the treating physician or nurse, which needs to be faxed to the Paxman Hub.
